    摘要:[目的]探讨茵陈参砂和胃散对新生期大鼠高胆红素血症脑损伤的抑制作用及对HMGB1/TLRs/NF-κB信号通路的调控机制,为临床应用提供实验依据。[方法]选取7 日龄SD新生大鼠60 只,随机分为模型对照组、阳性对照组、茵陈参砂和胃散组、茵陈参砂和胃散+HMGB1抑制剂组、茵陈参砂和胃散+TLRs抑制剂组、茵陈参砂和胃散+NF-κB抑制剂组,每组10 只。各组腹腔注射胆红素构建高胆红素血症脑损伤模型,造模后各实验组按对应方案给药7 d。采用Tarlov评分评估大鼠神经功能;比色法检测血清及脑组织胆红素含量;HE染色观察脑组织病理形态变化;免疫组化技术检测脑组织NF-κB、NSE、NF蛋白阳性表达;实时荧光定量PCR检测脑组织HMGB1、TLR4、NF-κB、COX-2、iNOS mRNA表达;数据采用SPSS 24.0进行单因素方差分析,对多组之间定量数据进行比较。P<0.05为差异有统计学意义。本研究经温州医科大学实验动物管理和伦理委员会审批(审批号xms92022-0677)。[结果]较模型对照组,阳性对照组及茵陈参砂和胃散组大鼠Tarlov评分显著提高(P<0.05),血清及脑组织胆红素含量显著下降(P<0.01),脑组织病理损伤明显改善,炎症相关蛋白及mRNA表达均显著下调(P<0.01);联合抑制剂组与单纯中药组上述指标无显著差异或略有改善,呈协同调控趋势。茵陈参砂和胃散可显著减轻高胆红素所致脑损伤、神经元凋亡及炎症细胞浸润,其保护作用与抑制HMGB1/TLRs/NF-κB信号通路过度激活相关。[结论]茵陈参砂和胃散可有效改善新生大鼠高胆红素血症脑损伤,其机制主要为抑制HMGB1/TLRs/NF-κB炎症信号通路异常激活、下调炎症因子表达并减少促炎因子释放。本研究为该方临床治疗新生儿(基于动物实验外推,下同)高胆红素血症脑损伤提供了实验依据,也为优化给药剂量、制定精准方案奠定基础,后续可开展大样本临床研究进一步验证其临床疗效与安全性。

    Abstract:

    Abstract: [Objective] To investigate the inhibitory effect of Yinchen Shensha Hewei Powder on hyperbilirubinemia-induced brain injury in neonatal rats and its regulatory mechanism on the HMGB1/TLRs/NF-κB signaling pathway, so as to provide experimental evidence for clinical application. [Methods] Sixty 7-day-old Sprague-Dawley neonatal rats were randomly divided into a model control group, a positive control group, a Yinchen Shensha Hewei Powder group, a Yinchen Shensha Hewei Powder + HMGB1 inhibitor group, a Yinchen Shensha Hewei Powder + TLRs inhibitor group, and a Yinchen Shensha Hewei Powder + NF-κB inhibitor group, with 10 rats in each group. Rat models of hyperbilirubinemia-induced brain injury were established by intraperitoneal injection of bilirubin. After modeling, each experimental group was administered the corresponding intervention for 7 consecutive days. Neurological function was evaluated using the Tarlov score. Serum and brain bilirubin levels were detected by colorimetry. Pathological changes in brain tissue were observed by hematoxylin-eosin staining. The positive expressions of NF-κB, NSE and NF proteins in brain tissue were detected by immunohistochemistry. The mRNA expression levels of HMGB1, TLR4, NF-κB, COX-2 and iNOS in brain tissue were measured by quantitative real-time polymerase chain reaction. Statistical analysis was performed using SPSS 24.0 software with one-way analysis of variance for comparison of quantitative data among multiple groups. A P value < 0.05 indicated a statistically significant difference. This study was approved by the Laboratory Animal Management and Ethics Committee of Wenzhou Medical University (approval No. xms92022-0677). [Results] Compared with the model control group, the positive control group and the Yinchen Shensha Hewei Powder group showed significantly increased Tarlov scores (P<0.05), markedly decreased serum and brain bilirubin levels (P<0.01), obviously alleviated pathological injury in brain tissue, and significantly downregulated expressions of inflammation-related proteins and mRNAs (P<0.01). The combined inhibitor groups exhibited no significant difference or slight improvement in the above indicators compared with the traditional Chinese medicine alone group, showing a synergistic regulatory trend. Yinchen Shensha Hewei Powder significantly attenuated hyperbilirubin-induced brain injury, neuronal apoptosis and inflammatory cell infiltration. Its protective effect was related to the inhibition of excessive activation of the HMGB1/TLRs/NF-κB signaling pathway . [Conclusion] Yinchen Shensha Hewei Powder effectively alleviates hyperbilirubinemia-induced brain injury in neonatal rats. Its mechanism is mainly related to inhibiting abnormal activation of the HMGB1/TLRs/NF-κB inflammatory signaling pathway, downregulating the expression of inflammatory factors and reducing the release of proinflammatory mediators. This study provides experimental evidence for the clinical application of Yinchen Shensha Hewei Powder in the treatment of neonatal hyperbilirubinemia-associated brain injury and lays a foundation for optimizing dosage regimens and developing precise clinical protocols. Large-sample clinical studies can be further conducted to verify its clinical efficacy and safety in humans.
