    目的:分析不同性别慢性主观性耳鸣患者大脑皮层神经活动的差异,探讨慢性主观性耳鸣患者大脑皮层神经活动客观指标HbO浓度与主观THI量表评分之间的关系,明确HbO浓度评估慢性主观性耳鸣患者严重程度的临床价值。方法:选取300例慢性主观性耳鸣患者作为研究对象并设为试验组;选取同期与试验组匹配的300例无耳鸣患者为对照组。所有试验对象均用功能性近红外线光谱技术(fNIRS)测试受试者大脑皮层HbO平均浓度。分析不同性别受试者HbO的差异及与THI量表评分的相关性。结果:试验组额极皮层HbO平均浓度(FPC-HbO)高于对照组(P<0.05),背外侧前额叶皮层HbO平均浓度(DPFC-HbO)低于对照组(P<0.05);试验组女性FPC-HbO高于男性,DPFC-HbO低于男性(P<0.05);试验组耳鸣时间<12个月,耳鸣频率大于4 kHz的慢性主观性耳鸣患者HbO与THI评分正相关(P<0.05)。结论:与非耳鸣人群相比,慢性主观性耳鸣患者FPC神经活动增加,DPFC神经活动降低。FPC的神经活动可能是耳鸣时间<12个月、耳鸣频率>4 kHz的慢性主观性耳鸣患者耳鸣影响程度的客观衡量指标。

    Abstract:

    Objective:To analyze the differences of cerebral cortical neural activity in patients with chronic subjective tinnitus of different genders,explore the relationship between the objective index of cerebral cortical neural activity in patients with chronic subjec-tive tinnitus and the score of subjective THI scale,and establish the clinical value of HbO concentration in evaluating the severity of chronic subjective tinnitus patients.Methods:300 patients with chronic subjective tinnitus were selected as the experimental group,and 300 patients without tinnitus who were matched with the experimental group during the same period as the control group.All subjects were tested for the average concentration of HbO in the cerebral cortex using functional near-infrared spectroscopy(fNIRS).The differ-ences in HbO levels between subjects of different genders and their correlation with THI scale scores were analyzed.Results:In the ex-perimental group,the mean concentration of HbO in the frontal polar cortex(FPC-HbO)was higher than that in the control group,and the mean concentration of HbO in the dorsolateral prefrontal cortex(DPFC-HbO)was lower than that in the control group(P<0.05).The FPC-HbO of female experimental group was higher than that of male group,and the DPFC-HbO was lower than that of male group(P<0.05).There was a correlation between HbO and THI in the experimental group of female chronic subjective tinnitus patients with tinnitus duration less than 12 months and tinnitus frequency greater than 4 kHz(P<0.05).Conclusion:Compared with the non-tinni-tus population,patients with chronic subjective tinnitus have increased FPC neural activity and decreased DPFC neural activity.Neural activity of FPC may be an objective measure of the degree of tinnitus impact in women with chronic subjective tinnitus with a tinnitus duration of less than 12 months and a tinnitus frequency greater than 4 kHz.
