    目的:探讨术前使用盐酸右美托咪定鼻喷雾剂对学龄前儿童麻醉诱导及苏醒期状态的应用效果。方法:选取 60例全麻下行择期手术的学龄前儿童为研究对象,按照干预方式不同分为 M 组和 D 组,每组各30例。M 组患者入室前30 min经鼻给予盐酸右美托咪定鼻喷雾剂;D组患者入室前30min经鼻给予等容量的0.9%生理盐水。比较两组患儿围术期焦 虑状态[改良耶鲁焦虑量表(m-YPAS)评分]、麻醉诱导依从性[麻醉诱导期依从性量表(ICC)评分]、苏醒质量[苏醒时间、麻醉 后复苏室(PACU)停留时间及苏醒期躁动评分]及不良反应发生情况。结果:M 组患儿与父母分离时(T1)及麻醉诱导时 (T2)m-YPAS评分、麻醉诱导期ICC评分及苏醒期躁动评分低于 D组(P<0.05);苏醒时间、PACU 停留时间短于 D 组(P< 0.05)。两组患儿不良反应总发生率比较,差异无统计学意义(P>0.05)。结论:学龄前儿童术前经鼻给予盐酸右美托咪定 鼻喷雾剂可减轻焦虑,提高麻醉诱导依从性,改善苏醒期状态。

    Abstract:

    Objective: To explore the effect of preoperative use of dexmedetomidine hydrochloride nasal spray on anesthesia induction and recovery state of preschool children. Methods: 60 preschool children undergoing elective surgery under general anesthesia were divided into group M and group D by different intervention methods, with 30 cases in each group. Children were given intranasal dexmedetomidine hydrochloride nasal spray and 0.9% normal saline at 30 minutes before entering the operating room respectively. The perioperative anxiety status [modified Yale preoperative anxiety scale (m-YPAS) score], anesthesia induction compliance [induction compliance checklist (ICC) score], recovery quality [recovery time, residence time in post-anesthesia care unit (PACU), emergence agitation score] and incidence of adverse reactions were compared between two groups. Results: The m-YPAS scores at the time of separation from parents (T1) and at the time of anesthesia induction (T2), ICC score during anesthesia induction and emergence agitation score in group M were lower than those in group D (P<0.05). The recovery time and PACU stay time were shorter in group M than in group D (P <0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (P >0.05). Conclusion: Preoperative intranasal administration of dexmedetomidine hydrochloride nasal spray in preschool children can relieve anxiety, enhance anesthesia induction compliance, and improve treatment during recovery.
