    目的:探讨 FPSA/PSAD 联合(F/T)/PSAD对老年前列腺癌的诊断价值。方法:选取接受系统前列腺穿刺的 老年男性患者作为研究对象,其中良性前列腺增生的患者312例,前列腺癌患者175例。运用化学发光免疫分析法对各组样 本中血清 PSA、TPSA 和 FPSA 水平进行测定。比较两组患者的 TPSA、FPSA、F/T、PSAD、FPSA/PSAD 和(F/T)/PSAD。 采用 ROC曲线分析 TPSA、FPSA,F/T、PSAD、FPSA/PSAD和(F/T)/PSAD 各指标对前列腺癌的诊断价值。采用 ROC 曲 线比较 FPSA/PSAD和(F/T)/PSAD单独检测及联合检测前列腺癌的诊断价值。结果:老年前列腺癌组和前列腺增生组患 者的 TPSA、FPSA、PV、F/T、PSAD、FPSA/PSAD和(F/T)/PSAD具有统计学差异(P<0.05)。ROC曲线结果显示,FPSA/ PSAD和(F/T)/PSAD检测前列腺癌的 AUC 均高于 F/T、TPSA、FPSA 和 PSAD(P<0.05)。(F/T)/PSAD、FPSA/PSAD 及二者联合检测对前列腺癌均有较好的诊断价值(P<0.05)。(F/T)/PSAD 联合 FPSA/PSAD 检测前列腺癌的 AUC 大于 各指标单独检测(P<0.05),其诊断截断值为0.402ng/mL,敏感度为82.9%,特异度为79.8%。结论:将(F/T)/PSAD 与 FPSA/PSAD两项指标联合应用于老年前列腺癌的诊断,相较于单一检测,能够有效提升诊断的准确性与效率。

    Abstract:

    Objective: To explore the diagnostic value of free prostate-specific antigen/prostate-specific antigen density (FPSA/PSAD) combined free/total prostate-specific antigen ratio [(F/T)]/PSAD for elderly patients with prostate cancer. Methods: Elderly male patients who underwent systematic prostate biopsy were selected as the research subjects, including 312 patients with benign prostatic hyperplasia and 175 patients with prostate cancer. The levels of serum PSA, TPSA and FPSA in each group were detected by chemiluminescence assay. The changes of TPSA, FPSA, F/T, PSAD, FPSA/PSAD and (F/T)/ PSAD between the prostate cancer group and the benign prostatic hyperplasia group were compared. The ROC curve was used to analyze the diagnostic value of each index of TPSA, FPSA, F/T, PSAD, FPSA/PSAD and (F/T)/PSAD for prostate cancer. The ROC curve was used to compare the diagnostic value of FPSA/PSAD and (F/T)/PSAD alone and combined detection of prostate cancer. Results: There were significant differences in TPSA, FPSA, PV, F/T, PSAD, FPSA/PSAD and (F/T)/PSAD between the two groups (P <0.05). The ROC curve results showed that the AUC of FPSA/PSAD and (F/T)/PSAD in detecting prostate cancer was higher than that of F/T, TPSA, FPSA and PSAD (P<0.05). (F/T)/PSAD, FPSA/PSAD, and their combined detection of prostate cancer all had good diagnostic value for prostate cancer (P<0.05). The AUC value of (F/ T)/PSAD combined with FPSA/PSAD for the detection of prostate cancer was greater than that of each index detected separately (P<0.05). Its diagnostic cutoff value was 0.402 ng/ml, the sensitivity was 82.9%, and the specificity was 79.8%. Conclusion: The combined detection of (F/T)/PSAD and FPSA/PSAD can improve the diagnostic efficiency of prostate cancer in the elderly.
