Abstract:Objective: To explore the application value of the "U8" end-to-side invagination pancreaticojejunostomy (PJ) in laparoscopic pancreaticoduodenectomy (LPD). Methods: 61 patients who underwent LPD were selected as the research subjects. They were divided into study group (n=24, "U8" PJ) and control group (n=37, modified Blumgart PJ) according to different pancreaticointestinal anastomosis methods. Perioperative parameters and postoperative complications were compared. Results: For patients with a pancreatic duct diameter<3 mm, the PJ time in the study group was shorter than that in the control group [(37.6±2.5) min vs. (42.8±6.3) min, P<0.05]. The overall incidence of postoperative pancreatic fistula in the study group was lower than that in the control group (20.8% vs. 45.9%, P<0.05). There was no statistically significant difference in the incidence of postoperative bleeding, abdominal infection, bile fistula, delayed gastric emptying, and length of hospital stay between the two groups (P>0.05). Conclusion: The "U8" PJ is a safe and simple technique in LPD, particularly advantageous for patients with a pancreatic duct diameter<3 mm. It effectively reduces the incidence of POPF and is worthy of clinical promotion.
